Photo & Video Release Consent

By submitting this form, I grant Performance Lab permission to use any photographs or videos of me taken by Performance Lab staff, as well as any photos or videos I voluntarily submit. I understand that these images may be used for marketing, promotional, and educational purposes, including but not limited to:

  • The Performance Lab website
  • Social media platforms
  • Digital and print advertising
  • Educational and promotional materials

I acknowledge that:

  • I am 18 years of age or older.
  • I will not receive compensation for the use of my image.
  • Photos/videos may be edited, altered, or modified for marketing purposes.
